Key Responsibilities
The Nurse Practitioner will manage patient panels in outpatient clinics and provide in-home clinical support to older adults. Responsibilities include performing wellness exams, coordinating continuity of care, and collaborating with an interdisciplinary team to optimize patient health outcomes.
Requirements
Candidates must hold a current Oregon Nurse Practitioner license and have at least 2 years of clinical experience in primary care, family medicine, internal medicine, gerontology, or palliative care. Additional requirements include active CPR/BLS certification, an Oregon driver's license, and proficiency in Medicare HCC coding.
