Key Responsibilities
The Advance Practice Provider is responsible for collaborating with physicians and family members to develop complex care plans aligned with the patient's health status and goals. This involves providing primary and urgent healthcare via telephone and tele video modalities to patients receiving home and community-based services.
Requirements
Candidates must possess an MS in Nursing, active national NP certification, and valid licenses in Texas, Kansas, or Iowa, along with an active RN Compact license. A minimum of two years of experience managing complex care cases and bilingual or multi-language skills are required.
