Key Responsibilities

The Care Transition Manager is responsible for assessing patients and coordinating effective discharge plans to ensure appropriate levels of care. They monitor patient risk scores daily and facilitate communication between patients, caregivers, and primary care physicians.

Requirements

Candidates must hold a Master's degree in Social Work and possess an active LMSW or LCSW license. At least one year of experience in discharge planning or care management is required, with a preference for three years of hospital-based social work experience.
