The Hospital Transition Coordinator manages care transitions for clients with complex medical and mental health needs between inpatient units and community services. They facilitate communication between providers, ensure proper follow-up scheduling, and advocate for client access to appropriate levels of care.
Candidates must hold a Bachelor's degree in human services and possess a valid driver's license. Two years of experience in a mental health setting is preferred.
