Key Responsibilities
Conduct in-home evaluations and develop individualized treatment plans to improve patient mobility and functional independence. Collaborate with physicians and the care team while documenting visits using the WellSky Kinnser EMR system.
Requirements
Requires an active Arizona Physical Therapist license, CPR certification, and at least one year of clinical experience. Candidates must have reliable transportation, a valid driver's license, and strong clinical judgment.
