Key Responsibilities

Conduct primary care visits in patient homes, assessing and diagnosing conditions for a senior and medically complex population. Manage patient care plans, including referrals, medication renewals, and advanced life care planning.

Requirements

Requires an APRN or PA-C degree with board certification and at least two years of primary care experience. Must possess a valid driver's license and insurance to perform home visits.
