Key Responsibilities
The RN Case Manager provides hands-on hospice nursing care and coordinates services for patients within their assigned community territory. They also collaborate with the clinical team to assess patient eligibility and provide ongoing support and education to patients and their families.
Requirements
Candidates must hold a current, unencumbered Registered Nurse license and have at least one year of experience in a medical-surgical or acute care setting. Proficiency with EMR systems, a valid driver's license, and current CPR certification are also required.
