Southern Oregon Wellness is a direct primary care practice focused on providing accessible, relationship-based healthcare. Our model allows clinicians to spend more time with patients, emphasize prevention and wellness, and develop long-term relationships rather than focusing on high-volume visits.

• Family Nurse Practitioner (FNP)
• Employment: Full-time or part-time
• Location: Southern Oregon
• Practice Model: Direct Primary Care

• What You’ll Do
• Provide comprehensive primary care to patients across the lifespan
• Diagnose and manage acute and chronic medical conditions
• Perform preventive health evaluations and wellness visits
• Develop individualized treatment and health-maintenance plans
• Provide patient education, counseling, and lifestyle guidance
• Build lasting relationships with patients and families
• Order and interpret appropriate laboratory and diagnostic studies
• Coordinate specialty care when needed
• Participate in improving the patient experience and practice processes
• Practice with an emphasis on prevention, whole-person care, and evidence-based medicine

• What We’re Looking For
• Current Family Nurse Practitioner (FNP) certification
• Active Oregon NP license and appropriate prescribing privileges
• Graduate of an accredited nurse practitioner program
• Current national board certification
• Strong clinical assessment and decision-making skills
• Excellent communication and interpersonal skills
• A genuine interest in relationship-based, preventive healthcare
• Comfortable working independently while collaborating with the broader care team
• Commitment to providing compassionate, personalized care
