Key Responsibilities

The social worker coordinates psychosocial activities and discharge planning in collaboration with the interdisciplinary care team to ensure continuity of care. They provide ongoing support, perform biopsychosocial assessments, and act as an advocate for patients and their families throughout the treatment continuum.

Requirements

Candidates must hold a Master’s Degree in Social Work and have at least 3 years of experience in a healthcare setting. Professional licensure such as LCSW, LMSW, or LPC is preferred, along with certification in case management.
