Key Responsibilities

The Social Worker coordinates discharge planning and manages the psychosocial needs of residents by acting as a liaison between them and the healthcare team. They also ensure compliance with facility policies, assist with insurance authorizations, and provide education on resident rights and advance directives.

Requirements

Candidates must hold a Bachelor's degree in a Human Services field such as Social Work, Psychology, or Rehabilitation Counseling. A minimum of one year of supervised social work experience in a healthcare setting and a valid CPR certification are required.
