Key Responsibilities
The Nurse Practitioner will provide clinical care and ongoing management for medically complex patients through telehealth and remote technologies. They will collaborate with an interdisciplinary team to develop individualized care plans and intervene early to prevent unnecessary hospitalizations.
Requirements
Candidates must hold a Master's degree or higher and possess a valid Nurse Practitioner license in Delaware or a bordering state. A minimum of 3–5 years of clinical experience as a Nurse Practitioner and certification as an FNP or Adult/Adult-Gerontology NP are required.
