The physical therapist will perform clinical assessments, including OASIS, and develop individualized plans of care for geriatric patients in their homes. They are responsible for documenting clinical notes, providing skilled therapy treatments, and supervising physical therapy assistants and home health aides.
Candidates must hold a current physical therapy license and possess strong clinical assessment skills. At least one year of home health experience is preferred, along with the ability to manage an independent, fast-paced schedule.
