Key Responsibilities

The ambulatory clinical pharmacist ensures safe, efficacious, cost-effective, and high-quality medication therapy disease management for patients in the ambulatory setting, working under a collaborative practice agreement with physicians. Responsibilities include direct patient care, consulting on medication-related problems, assessing therapy regimens, and titrating/managing medications to optimize clinical outcomes.

Requirements

Candidates must possess a Bachelor's Degree in Pharmacy and be a Licensed Pharmacist in Pennsylvania within 90 days of hire. The role requires proficiency in medication and disease management, the ability to gather necessary clinical data, and skills in educating both patients and healthcare professionals.
