Job Description
Full-time post-acute Hospitalist needed for Skilled Nursing Facility (SNF). Clinical duties involve hospitalist work w/admitting, discharging patients & seeing them on a daily basis. These patients have been transferred for acute rehab services after hospital stay. Support staff includes PM&R physician & an experienced APP team.
Work schedule consists of M-F from 8am-5pm; physician can leave early once work is complete but must remain available by phone until shift ends at 5pm. See an average of 15 patients per day. No night shifts required however share night coverage w/other Hospitalists from home. Key Responsibilities
The hospitalist will manage patient admissions, discharges, and daily clinical rounds at a skilled nursing facility. They will also share night coverage responsibilities with other hospitalists from home.
Requirements
The position requires a qualified physician to provide acute rehab services for patients transferred from hospitals. Candidates must be available for a Monday through Friday schedule and participate in an on-call rotation.
