Licensed Clinical Social Worker

Key Responsibilities

The Social Worker routinely assesses participant and family needs to determine appropriate social services interventions, coordinating with the Interdisciplinary Team (IDT) to implement the plan of care and provide psychosocial support. This role involves acting as an education resource for the team while supporting the goal of high-quality, person-centered care for all participants.

Requirements

Candidates must possess a Master’s Degree in Social Work and be licensed as a Licensed Clinical Social Worker in Kentucky, along with current CPR certification and medical clearance. A minimum of one year of experience working with frail or elderly populations and having reliable transportation are also mandatory requirements.
