Mental Health Therapist Team Lead for Community Support Team

The CST Team Lead is a full-time licensed clinician responsible for providing clinical leadership, direct services, and oversight of the Community Support Team. This position helps ensure services are person-centered, recovery-focused, clinically appropriate, and provided in accordance with NC Medicaid Clinical Coverage Policy 8A-6 – Community Support Team (CST) and agency requirements.

Job Summary

We are seeking an experienced and motivated CST Team Lead to provide clinical leadership to our Community Support Team.

The Team Lead coordinates service delivery, provides individual therapy and clinical interventions, completes and reviews assessments, guides treatment planning, supervises team activities, and ensures staff are assigned according to each member's clinical needs.

This position provides services in the office and community and works closely with members, families, healthcare providers, hospitals, community agencies, and other treatment professionals.

Essential Duties and Responsibilities

• Provide clinical leadership and oversight of the Community Support Team.
• Coordinate and drive the delivery of CST services.
• Provide individual therapy and other clinical interventions within scope of practice.
• Complete and coordinate assessments and reassessments of members' clinical needs.
• Participate in the development, implementation, and review of person-centered plans and crisis plans.
• Assign CST staff based on ember needs and staff expertise.
• Facilitate team meetings, case consultations, and treatment reviews.
• Provide clinical guidance to team members regarding complex cases and crisis situations.
• Monitor member progress and recommend changes in interventions or level of care when appropriate.
• Provide community-based interventions that promote stability, independence, and successful community living.
• Address mental health, substance use, housing, medical, social, vocational, and other identified needs.
• Utilize evidence-based and trauma-informed interventions appropriate to the member's needs.
• Assess and respond to psychiatric crises, suicide risk, substance-related crises, and other safety concerns.
• Develop and review individualized crisis and safety plans.

Coordinate care with psychiatrists, primary care providers, therapists, hospitals, care managers, housing providers, and community agencies.

• Support transitions following hospitalization, crisis services, or other levels of care.
• Ensure clinical documentation is accurate, timely, and supports medical necessity.
• Review progress notes, treatment plans, assessments, crisis plans, and other required documentation.
• Participate in clinical audits, quality-improvement activities, and utilization reviews.
• Maintain compliance with HIPAA, 42 CFR Part 2, NC Medicaid requirements, and agency policies.
• Complete administrative and supervisory duties related to the role.

Communication & Coordination

Communicate effectively with members, families, providers, hospitals, care managers, DSS, and community agencies.

• Serve as a clinical liaison between members, CST staff, providers, and community partners.
• Maintain professional communication and appropriate boundaries.
• Participate in interdisciplinary meetings and communicate significant changes in members' clinical needs.
• Respond professionally and appropriately during crisis situations.

Qualifications

Master's degree in social work, counseling, psychology, marriage and family therapy, or another qualifying behavioral health field.

Current North Carolina professional license that meets CST Team Lead requirements, including:

• LCSW or LCSWA
• LCMHC or LCMHCA
• LMFT
• Licensed Psychologist
• Licensed Psychological Associate (LPA)

Associate-level licensed clinicians may qualify when actively pursuing full licensure and must obtain full licensure within 30 calendar months from the date of hire, consistent with NC Medicaid CST requirements.

Minimum one year of relevant experience working with the population served.

Experience with serious mental illness, substance use disorders, co-occurring disorders, crisis intervention, or community-based behavioral health services preferred.

• Previous supervisory or team leadership experience preferred.
• Strong clinical assessment, documentation, communication, and organizational skills.
• Ability to provide effective leadership within a multidisciplinary team.
• Valid driver's license and reliable transportation for community-based services.
• CPR/First Aid certification or ability to obtain certification within the required timeframe.
