Registered Nurse (RN) Case Manager – Home Health | South Dallas-Fort Worth & Surrounding Areas

Ultimate Home Health Care is seeking dedicated RN Case Managers to provide high-quality home healthcare to clients across the Dallas-Fort Worth metroplex and surrounding areas. Role Responsibilities
- Provide professional, compassionate care in clients’ homes
- Manage diverse client needs with Home Health expertise
- Document care accurately using OASIS/Kinnser
- Respond to PRN emergency calls as needed
Qualifications
- Graduate of an accredited Nursing Program
- Active Texas RN License and Driver’s License
- Minimum 1 year of Home Health experience
- Experience with OASIS/Kinnser preferred
- Ability to work flexible schedules and travel locally
