Nurse Practitioner - Hospice and Palliative Care Services

Responsibilities

  • Proactive, collaborative team member in a hospice care environment
  • Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care
  • Commitment to caring for patients and partners
  • Complete all documentation and records in a timely and accurate manner
  • Follow established policies and procedures as related to the scope of practice
  • Maintain continuity of patient care in a hospice setting

Licensure, Certification, and Educational Requirements

Current, active, and unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licensure in state of practice

Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care

Requirements

Candidates must hold a current, active, and unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) license in the state of practice. Additionally, board certification by the AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care is required.
