Key Responsibilities
Provide advanced primary and specialty care to geriatric and medically complex patients in their homes or care settings. Lead and mentor Nurse Practitioners and Physician Assistants while coordinating care with interdisciplinary teams to improve patient outcomes.
Requirements
Requires a Doctor of Medicine or Osteopathy degree with an unrestricted state license and board certification in family practice, internal medicine, geriatric, or palliative care. Candidates must have prior supervisory experience and the ability to travel daily within the service area.
