Key Responsibilities

The Advanced Practice Provider will conduct comprehensive patient assessments and develop individualized palliative care plans to manage symptoms for patients with serious illnesses. They will also collaborate with an interdisciplinary team to provide holistic care and facilitate discussions regarding goals of care and end-of-life options.

Requirements

Candidates must hold a Master’s degree from an accredited PA program or a master’s level advanced practice nurse program. An active APRN or PA license in the State of Illinois and an active DEA license are required.
