About the Role

Join Carefirst Seniors & Community Services Association as a Nurse Practitioner (Community Health) in United States.

Key Responsibilities

  • Based & Mobile Service Delivery: Deliver care in community and non
  • traditional settings, including mobile clinics and outreach locations. Adapt care approaches to address social, cultural, and environmental factors impacting client health and access to services.
  • Interprofessional Collaboration: Collaborate with an interprofessional team to support coordinated, integrated care planning. Participate in team huddles, case discussions, and shared decision
  • making to optimize client outcomes.

Requirements

  • Baccalaureate in nursing or equivalent from an accredited university (Master’s degree in nursing preferred) RN with Extended Class required.
  • Current Certificate of Competence with the college of Nurse of Ontario.
  • 5 years related clinical and/or management experience, preferably in a community healthcare setting or combination of community and hospital or public health settings considered an asset.
  • Skills/experience to fulfill major functions of the role including wellness care, assessment, and diagnosis.

Benefits & Compensation

  • Salary range: $100,000 to $145,000 annually.
