Nurse Practitioner (Community Health)

Carefirst Seniors & Community Services Association is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area.

With a staff of more than 550 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are “home-bound” and frail.

We are currently seeking a Nurse Practitioner to fill a vacant position at our Community Health team.

About the Community Health Team

The Carefirst Community Health Team supports population health by bringing preventive care, health education, screening, and system navigation directly into the community, with a focus on equity and early intervention. By working alongside community partners, the team helps identify unmet needs, reduce barriers to care, and connect individuals to the right health and social supports at the right time.

The Community Health Team delivers culturally appropriate, community-driven and equity-focused programs, including:

• Preventive Care Clinic
• Cancer Prevention Program
• Mobile Health Clinics
• Black Health Initiative

These programs aim to improve equitable access to care, promote wellness, and strengthen connections within diverse communities.

Terms: Casual

Location: 300 Silver Star Blvd., Scarborough and community locations in the GTA

Pay range: $100,000 - $145,000

Responsibilities

• Clinical Care & Assessment: Provide comprehensive and focused health assessments, diagnosis, and management of acute and chronic conditions within NP scope of practice. Order and interpret diagnostic tests, prescribe medications, and initiate treatment plans in accordance with legislation, standards, and organizational policies.
• Community-Based & Mobile Service Delivery: Deliver care in community and non-traditional settings, including mobile clinics and outreach locations. Adapt care approaches to address social, cultural, and environmental factors impacting client health and access to services.
• Interprofessional Collaboration: Collaborate with an interprofessional team to support coordinated, integrated care planning. Participate in team huddles, case discussions, and shared decision-making to optimize client outcomes.
• Health Education & Client Empowerment: Provide patient-centred education, counselling, and self-management support using trauma-informed and culturally safe approaches. Support shared decision-making and client goal-setting based on client needs and health literacy.
• Documentation & Quality Standards: Maintain accurate, timely, and compliant clinical documentation in the EMR. Adhere to organizational policies, clinical guidelines, and regulatory standards, and participate in quality improvement activities as required.
• Program & Population Health Support: Support population-health initiatives by contributing clinical expertise to prevention, screening, and chronic disease management programs. Identify emerging client needs and service gaps to inform program planning and improvement.
• Professional Accountability: Practice in accordance with College of Nurses of Ontario standards and Nurse Practitioner competencies. Maintain required licensure, certifications, and ongoing professional development

Requirements

• Baccalaureate in nursing or equivalent from an accredited university (Master’s degree in nursing preferred) RN with Extended Class required.
• Current Certificate of Competence with the college of Nurse of Ontario.
• Minimum 3-5 years related clinical and/or management experience, preferably in a community healthcare setting or combination of community and hospital or public health settings considered an asset.
• Skills/experience to fulfill major functions of the role including wellness care, assessment, and diagnosis.
• Proven ability to coordinate care collaboratively with other interdisciplinary team members as well as to effectively function independently with clients.
• Demonstrated ability to relate therapeutically with clients. Includes strong negotiation and conflict resolution skills.
• Facilitation skills for education and promotion of health programs.
• Demonstrated use of theory and research/evidence-based outcomes in practice.
• Familiar with electronic medical records (EMR) systems
• Proficiency in additional languages (e. g., Cantonese, Mandarin, or other community languages) is an asset.
• Proficient in Microsoft Office including Excel
• Ability to travel to community sites and outreach locations as required
• Ability to accurately document, track basic data, and support reporting requirements. to vehicle, driver's license
• Current CPR certification (Level C or HCP) an asset
• A valid vulnerable sector police record check completed within one year
