Key Responsibilities
The Social Worker performs psychosocial, emotional, and bereavement assessments to develop and implement goal-directed care plans for hospice patients. They also participate in interdisciplinary team conferences and provide ongoing counseling to patients and families regarding end-of-life issues.
Requirements
Candidates must hold a Master of Social Work (MSW) degree from a CSWE-accredited school. One year of experience in home health care and hospice experience are preferred, along with a valid driver's license and reliable transportation.
