Key Responsibilities

The Clinical Consultant Pharmacist maintains continuity of care by collaborating with onsite physicians, nursing staff, and other health providers at various congregate living communities. Responsibilities include completing clinical pharmacy activities, conducting medication reviews, and providing education aligned with provincial standards and site policies.

Requirements

Candidates must be licensed and insured with the Ontario College of Pharmacists or equivalent provincial bodies, possess a valid driver's license, and have a complete understanding of provincial regulations governing LTC homes and Retirement Communities. Essential skills include strong partnership fostering, excellent organization, critical thinking, and self-directed work ethic.
