Key Responsibilities

The Clinical Pharmacist will deliver evidence-based care within a multidisciplinary team to optimize medication therapy and improve patient outcomes. They will manage complex medication regimens, perform clinical assessments, and provide patient education on chronic health conditions.

Requirements

Candidates must hold a degree from an accredited school of pharmacy and possess an active pharmacist license. Preferred qualifications include a Pharm.D. with specific ambulatory care practice experience or completion of a relevant pharmacy residency.
