Perform ongoing clinical assessments, including OASIS, and develop individualized plans of care for geriatric patients in their homes. Provide skilled physical therapy treatments, supervise therapy assistants, and maintain accurate clinical documentation.
Requires a current Physical Therapist license and strong clinical assessment skills. At least one year of home health experience is preferred, along with the ability to manage an independent schedule.
