The Nurse Navigator assesses patient needs to develop and implement comprehensive care plans while facilitating seamless transitions between inpatient and outpatient settings. This role serves as a liaison between patients, families, and the healthcare team to improve patient flow, eliminate barriers, and ensure high-quality, cost-effective care.
Candidates must hold a Bachelor's Degree in Nursing and possess at least 3 years of relevant nursing experience. Current Michigan RN licensure and BLS certification (AHA or ARC) are required upon hire.
