• ptoms, prescribes medications, and develops or updates individualized care plans. They work closely with patients and families to understand goals of care, explain treatment options, and provide guidance throughout the hospice or palliative care journey. The Nurse Practitioner also collaborates with physicians and the interdisciplinary team—including nurses, social workers, chaplains, and other care professionals—to ensure care is coordinated and aligned with each patient’s wishes.

Qualifications

• ptom management, pain management, and end-of-life care Experience discussing goals of care and advance care planning with patients and families Experience working within an interdisciplinary team Familiarity with Medicare hospice regulations and hospice eligibility requirements Experience providing care in patients’ homes, long-term care facilities, or other community-based settings What’s in it for you?
