Key Responsibilities
Provide one-on-one physical therapy to older adults in their homes or senior living communities to improve mobility and functional independence. Develop individualized plans of care, document effectively in the EMR, and collaborate with an interdisciplinary team.
Requirements
Must hold a valid Physical Therapy license or be eligible to obtain one in the state of practice. Candidates must possess a degree from an accredited Physical Therapy program and be able to meet the physical demands of in-home care.
