Work Schedule

The provider will deliver longitudinal, holistic primary care to medically complex patients within their homes and communities. They will manage chronic and acute conditions while coordinating with an interdisciplinary team to improve health outcomes and reduce hospital readmissions.
Requirements
Candidates must be a Nurse Practitioner or Physician Assistant with an active Colorado license and a master's degree. The role requires a passion for geriatric care and the ability to work independently in home-based environments.
