The Physical Therapist Case Manager will evaluate and treat patients in their homes to restore function and independence while coordinating care with physicians. They are also responsible for supervising Physical Therapy Assistants and ensuring all care plans meet performance standards.
Candidates must hold a current, unrestricted California Physical Therapist license and a valid driver's license. Experience in home health care and supervising Physical Therapy Assistants is highly preferred.
