This position offers weekends off. Work with Crescent Hospice in Summerville, South Carolina, United States.
The RN Case Manager will deliver hands-on hospice nursing care and coordinate services for patients within the community. They will also partner with the Clinical Director to assess patient eligibility and provide ongoing support and education to families.
Candidates must hold a current unencumbered Registered Nurse license and have at least one year of experience in a medical-surgical or acute care setting. Proficiency in EMR systems and a valid driver's license with reliable transportation are also required.
