Key Responsibilities

The Physical Therapist is responsible for evaluating, planning, and implementing therapy interventions for home health patients. They must also collaborate with the interdisciplinary team to manage patient care, documentation, and discharge planning.

Requirements

Candidates must hold a current state licensure in Physical Therapy in good standing and possess a Bachelor's degree. One or more years of experience in long-term care is preferred.
