The primary function is to provide coordinated care in the home for patients of all age groups, involving psychosocial assessments, analysis, counseling, and referrals to meet patient/family needs. Essential functions include assessing hospice patients for psychosocial, financial, environmental, and community needs, and collaborating with the Interdisciplinary Team for coordinated care planning.
Candidates must possess a Master's Degree in Social Work from a State accredited program and have one year of social worker experience in a healthcare agency. Required qualifications also include reliable transportation, valid auto liability insurance, and the ability to work under stressful conditions while meeting patient/family needs.
