Key Responsibilities

Provide one-on-one physical therapy to older adults in their homes or senior living communities to improve mobility and functional independence. Collaborate with an interdisciplinary team and document care effectively within the EMR system.

Requirements

Must hold a valid Physical Therapy license or be eligible to obtain one and possess a degree from an accredited Physical Therapy program. Candidates must be able to perform physical tasks such as lifting and patient transfers, and maintain current CPR certification.
