Key Responsibilities

The Social Worker will assess the psychosocial status of patients and families related to terminal illness, providing necessary counseling, crisis intervention, and education on topics like advanced directives. Responsibilities also include participating in the individualized plan of care, acting as a liaison with community agencies, and assisting with practical, financial, and legal planning needs.

Requirements

Applicants must possess a Master's degree in social work, or a Bachelor's degree in Social Work, along with a minimum of two years of supervised experience in healthcare, with hospice experience being preferred. A strong understanding of hospice philosophy and issues surrounding death and dying is required.
