Diabetes Nurse Practitioner

Key Responsibilities

The Nurse Practitioner will function interdependently within the hospital to provide holistic care focused on glycemic management, including assessing new and existing consults, writing necessary orders, and managing physical, psychosocial, education, and discharge needs. This role involves collaborating with the diabetes self-management education team and other hospital staff to coordinate care and revise patient plans as needed.

Requirements

Candidates must possess a Master's or Doctoral Nursing degree as a Family or Acute Care Nurse Practitioner with a minimum of two years of experience in that role. A current state license, DEA license, and Board Certification in Advanced Diabetes Management (BC-ADM) are required, with the latter needing to be obtained within two years if not already credentialed.
