Key Responsibilities
The specialist will conduct behavioral health intakes and psychosocial assessments while providing clinical oversight within skilled nursing and long-term care settings. They will also support value-based care initiatives and serve as a clinical resource escalation point for the regional team.
Requirements
Candidates must hold a Master's degree in social work, professional counseling, or marriage and family therapy and possess a valid LPCC, LISW, or IMFT license. A minimum of three years of clinical experience and Medicare eligibility are required for this role.
