The Palliative Care Nurse Practitioner provides direct patient and family care through consultations for individuals with serious illnesses. They are responsible for conducting comprehensive assessments, formulating care plans, and performing medication reconciliation to ensure optimal patient outcomes.
Candidates must hold an RN/APRN license with prescriptive authority and possess at least 5 years of clinical nursing experience. A current BCLS/CPR certification is required, and experience in hospice, palliative care, or oncology is preferred.
