The Team Clinician provides comprehensive rehabilitation and case management services to individuals with severe and persistent mental illness as part of a multidisciplinary PACT team. Responsibilities include conducting psychiatric assessments, developing treatment plans, and providing ongoing support for activities of daily living.
Candidates must hold a Master's degree in a human service discipline and possess a valid LCSW, LICSW, or LMHC license in Massachusetts. Five years of supervised clinical or rehabilitation experience and a valid driver's license with a reliable vehicle are required.
