Care Management Extender - Ambulatory - Family Medicine Center

Key Responsibilities

The Care Management Extender provides clerical and administrative support to RN and SW Case Managers to facilitate patient care transitions and plan execution. Responsibilities include scheduling appointments, managing patient records, and coordinating with alternate care facilities and home health services.

Requirements

Candidates must possess a High School Diploma or GED and have at least 2 years of healthcare experience. A Basic Life Support (BLS) certification from the American Heart Association is required for this position.
