Job Description
Physical Therapist at Providence Saint John’s Health Center in Santa Monica, CA.
This position is Fulltime and will work 8-hour, Day shifts.

The Physical Therapist supports the mission, vision, values and objectives of
Saint John's Health Center. Responsibilities include, but are not limited to,
discharge planning, performing patient evaluations, establishing and
implementing a plan of care, monitoring patient's progress.

Required Qualifications
* Bachelor’s Degree with certificate from an accredited program in Physical
Therapy.
* Upon hire: California Physical Therapist License
* Upon hire: National Provider BLS - American Heart Association

position. The successful candidate's placement on the wage range for this
position will be determined based upon relevant job experience and other
applicable factors. These amounts are the base pay range; additional
compensation may be available for this role, such as shift differentials,
standby/on-call, overtime, premiums, extra shift incentives, or bonus
opportunities.

The Physical Therapist is responsible for performing patient evaluations, establishing and implementing individualized plans of care, and monitoring patient progress. They also coordinate discharge planning to support the mission and objectives of the health center.
Requirements
Candidates must hold a Bachelor’s degree from an accredited Physical Therapy program and possess a valid California Physical Therapist License. Additionally, a National Provider BLS certification from the American Heart Association is required upon hire.
