Key Responsibilities

  • provide whole person care addressing bio-psycho-social needs of patients and family.
  • Provide patients/families with the psychosocial support needed to cope with chronic, acute or terminal illne.
  • provide education about homecare, palliative care, and hospice.
  • provide proof of license within 4 months of start.

Requirements

  • MN must have one of the following:
  • Licensed Independent Social Worker (LISW) or
  • Licensed Independent Clinical Social Worker (LICSW)
