Job Description
Park Nicollet Methodist Hospital is looking to hire a Social Worker/Care Manager
to join our team! Come join us as a Partner for Good and help us make an impact
on the care and experience that our patients and their families receive every
day.
The Social Work Care Manager provides patient-centered care management services
to patients and families in an inpatient hospital setting. This position serves
as an integral member of the interdisciplinary healthcare team and is
responsible for identifying care coordination needs in collaboration with the
inpatient and ambulatory care teams, along with the patient and the patient’s
external support systems.
Within the context of an interdisciplinary team, the Social Work Care Manager:
* Participates in progressing patients toward discharge/next level of care by
identifying barriers, implementing appropriate interventions.
* Securing transition plans that are aligned with shared goals of care.
The Social Work Care Manager will consistently and thoughtfully apply social
work values and continuous quality improvement in daily work.
Schedule: FTE.8 (32 hrs. weekly) 8 block schedule between M-F 8a-4:30p and
every 4th weekend rotation.
Required Qualifications
Education, Experience or Equivalent Combination
* BSW – Bachelor’s of Social Work
Licensure/ Registration/ Certification
* Current Minnesota license to practice as LSW
