Key Responsibilities

Provide evaluative, consultative, and treatment interventions to home health patients in their place of residence. Collaborate with the interdisciplinary team and attending physician to develop and manage individualized patient care plans.

Requirements

Must hold a valid professional license in the state of practice and be a graduate of an ASHA accredited school. Prior home health experience is preferred.
