Matrix Home Care, LLC, as manager for Gulfside Home Health, LLC, AHCA License #20255096, is currently hiring qualified caregivers and clinicians to support Gulfside patients and services. Employees hired for these roles will be employees of Matrix Home Care, LLC.

Position Summary
Matrix Home Care is seeking experienced and compassionate Physical Therapists (PTs) for home health opportunities in Pinellas County. This position is ideal for therapists who are dependable, patient-focused, and comfortable providing therapy services in the home setting.
The Physical Therapist will evaluate patients, develop individualized treatment plans, provide skilled therapy services, and help patients improve mobility, strength, balance, safety, and overall independence in the home.

Responsibilities
Complete physical therapy evaluations and reassessments in the home setting.
Develop and implement individualized treatment plans based on patient needs and physician orders.
Provide skilled therapy services to improve mobility, balance, strength, endurance, safety, and functional independence.
Educate patients and caregivers on home exercise programs, safety precautions, fall prevention, and mobility techniques.
Monitor patient progress and update the plan of care as needed.
Communicate with physicians, clinical leadership, and the care team regarding patient status and changes in condition.
Complete accurate and timely documentation.
Follow company policies, home health standards, and all applicable state and federal regulations.

Requirements
Active Florida Physical Therapist (PT) license in good standing.
Previous home health experience preferred.
Experience with evaluations, treatment planning, patient education, gait training, balance training, strengthening, and fall prevention preferred.
Must be reliable, professional, compassionate, and patient-focused.
Must be comfortable working independently in the field.
Strong communication and documentation skills are required.
Must be able to pass all required background screening and onboarding requirements.
