The Hospice Triage Registered Nurse will perform telephone triage to assess patient conditions, manage symptoms, and coordinate care with the interdisciplinary team. They will also handle new hospice referrals, provide clinical guidance to caregivers, and ensure accurate documentation of all triage activities.
Candidates must be a graduate of an accredited School of Nursing and hold a valid Registered Nurse license in the state. At least one year of hospice experience, knowledge of Medicare/Medicaid guidelines, and a current CPR (BLS) certification are required.
