The Hospice Triage Registered Nurse will perform comprehensive telephone triage to assess patient conditions, symptoms, and psychosocial needs while coordinating care with the interdisciplinary team. They will also manage new hospice referrals, provide clinical guidance to caregivers, and determine the necessity for in-person visits or emergency interventions.
Candidates must be a graduate of an accredited School of Nursing and hold a current Registered Nurse license in the state. At least one year of hospice experience, knowledge of Medicare/Medicaid guidelines, and a current CPR/BLS certification are required.
