Key Responsibilities
The pharmacist is responsible for providing safe and effective medication therapy to hematology/oncology patients while collaborating with multidisciplinary teams. They also lead formulary management, participate in clinical rounds, and conduct quality improvement initiatives.
Requirements
Candidates must hold a Doctor of Pharmacy (Pharm.D.) degree and possess at least one year of relevant registered pharmacist experience. A valid Florida pharmacist license and BLS certification are required, with residency training and board certifications preferred.
