Cardiology Structural Heart Clinical Manager- NP/PA

Purpose/Summary: The Cardiology Structural Heart Clinical Manager will support the Director of Cardiology Acute Care and Ambulatory Advanced Practice Providers in ensuring successful operations, fiscal management, and quality of the structural heart program at Hudson Valley Cardiology Practice.

This leader is responsible for overseeing day-to-day clinical and operational functions while providing effective people management and employee relations support to the care team. The incumbent will foster a high-performing, engaged workforce by promoting accountability, professional development, and a collaborative team environment The leader will also demonstrate and champion the organizations mission, vision, values, and service excellence standards, helping to build and sustain a culture focused on exceptional outcomes in patient safety, clinical quality, patient experience, and team member engagement and satisfaction.

Note: Role is 75% clinical/25% administrative/leadership

Essential Responsibilities

1. Operational Duties: a. Facilitate team coordination of duties with regular review of processes, priorities, and efficiency. b. In collaboration with the program director, arrange opportunities for outreach and public education. c. Coordinate work schedules for Structural Heart APP and RN clinicians. d.

Manage PTO requests and coverage for structural heart service APPs and RNs. e. Tracking time and attendance, and approving payroll. f. Assists with day-to-day operations such as IT access and issues, sick call coverage, etc. g. Participates in interview process of new candidates.

Participates and helps to coordinate precepting and training of new clinical team members and students. h. Assists with execution of operational objectives and strategic priorities in the creation and achievement of goals. i.

Provide active people management and employee relations support by coaching, mentoring, and developing team members to achieve optimal performance. Consistently monitor performance, provide real-time feedback, and ensure understanding of expectations.

Lead through influence while fostering a collaborative and respectful team environment. j. Address employee relations matters proactively, including performance concerns, interpersonal conflicts, and behavioral issues.

Partner with HR and leadership as appropriate to ensure fair, consistent, and compliant resolution. k. Helps to develop, monitor, and modify protocols, procedures, and processes to streamline patient care and improve workflows. l.

Works with clinical leadership to adopt and implement quality initiatives and outcome reporting processes specific to structural heart program. m. Effectively communicates with colleagues across the system.

Participates in hospital, ambulatory, HVI, and/or network-focused operational and clinical committees as relevant and appropriate. n. Ability to identify issues, perform gap analysis, prioritize business needs and execute solutions. o. Escalates issues and concerns proactively to Director as needed. p. 2. Clinical Duties: a. Coordinate all services and systems required for an organized multi-disciplinary team approach to the evaluation, treatment, and follow-up of patients requiring structural heart procedures including TAVR, Mitra-Clip, TMVR, TTVR, Watchman device, and other related procedures. b.

Coordinate material, plan agenda, and attend weekly multidisciplinary structural heart program meetings. c.

Attend weekly valve clinic with physicians, which includes a thorough assessment of patients involving clinical evaluation, psychological evaluation, surgical risk scoring, and frailty and mobility assessments, in order to make an accurate determination of the patient�s qualification for the procedure. d.

Perform prn and follow-up office visits for structural patients before and after their procedure. e. Receive incoming referrals from providers and respond in a timely manner to ensure rapid access to the structural heart screening process.

Triages referrals according to criteria to assure patient needs are met on a priority level. f. Orders and manages clinical tests according to protocol to complete assessment process (CTA, echocardiograms, TEE, PFT, angiograms, etc.). g.

Initiates and provides outside referrals to specialists for patients as needed for evaluation. h. Coordinates structural procedure schedule and communicates patient flow with providers and staff as needed. i.

Educates patient and family about the referral, screening process, steps of evaluation, procedure scheduling, and follow up. Keeps patient and family informed throughout the entire process. j. Communicates to referring providers the plans for patient evaluation and procedures. k.

Serve as the content expert for structural procedures and assist other staff who care for patients as requested to ensure that quality care is delivered in a consistent fashion. l.

Work closely with vendor clinical specialists for updates to device changes and training for internal staff, as well as provide case planning for individual patient care. m. Develop and revise policies and procedures with program director surrounding structural patient care. Implement and educate internal staff. n.

Participate in TVT Registry for collection of data and quality improvement. o. Interface with Research Department for patients who may be candidates for participation in structural heart related trials. p. Work with IT to develop/revise electronic medical record orders for evaluation and clinical care. q.

Provide information on hospital and professional billing to applicable personnel to ensure billing compliance.

• Master degree or higher in nursing or physician assistant studies
• Minimum of three (3) years of cardiology APP experience, or equivalent experience
• Lead APP or supervisory and/or project management experience

License, Registration, or Certification Requirements

• New York State Nurse Practitioner License or Physician Assistant License
• National Board Certification as a Nurse Practitioner or Physician Assistant
• Advanced Cardiac Life Support (ACLS)
