Position Summary Impact Health PC is seeking a Medical Director and/or Attending Physician to provide care within Skilled Nursing Facilities in the Litchfield, CT area.
This position offers the opportunity to provide direct patient care, clinical leadership, or a combination of both, depending on experience and facility needs.
You will work closely with facility leadership, advanced practice providers, and interdisciplinary teams to ensure high-quality patient care and clinical excellence.
Key
Responsibilities
Provide comprehensive medical care to residents in Skilled Nursing Facilities. Serve as an Attending Physician and/or Medical Director based on facility needs. Provide clinical leadership, medical oversight, and guidance to facility staff and providers.
Collaborate with nursing staff, advanced practice providers, and interdisciplinary care teams. Review patient care plans, clinical outcomes, and quality improvement initiatives. Participate in Quality Assurance and Performance Improvement (QAPI) programs.

About Us
Impact Health PC impacthealthpc.com Qualifications: MD or DO from an accredited medical school. Active, unrestricted Connecticut medical license. Board Certified or Board Eligible, preferably in Internal Medicine, Family Medicine, or Geriatrics.
Current state and federal prescribing registrations, including controlled substance authority as required. Eligible to participate in Medicare and Medicaid programs. Experience in Skilled Nursing, Long-Term Care, Geriatrics, or Hospital Medicine preferred.
Strong clinical leadership, communication, and organizational skills. Knowledge of long-term care regulations and quality standards preferred. #HP123
